Disease Management

Disease Management is a system developed for coordinating the healthcare services and the related communication as well as for the follow-up of chronic diseases that require the patients to undertake their self-care.

When it is considered that the chronic diseases threat human life, reduce the quality of life and the costs required for the treatment of such diseases are rather high; the disease management programs developed for fighting against those become crucial. 

According to the data of World Health Organization, chronic diseases are the cause of 60% of overall deaths. Furthermore, approximately 75% of the healthcare expenditures in almost all countries, including Turkey, are being spent for chronic diseases. According to the latest studies, approximately 24 million people in Turkey suffer from chronic diseases. The distribution of that 24 Million amongst the chronic diseases is as listed below;

  • Hypertension: 15 Million
  • Diabetes: 4 Million
  • COPD: 3 Million
  • Cardiovascular diseases: 2 Million

Potential side effects of the diseases may be avoided or retarded by means of Disease Management Programs. The objective of this service being provided with the approach “Don’t let the disease control you, you control the disease” is providing the maintenance of healthy life of individuals despite their diseases.

Medical Center’s Disease Management Programs

Disease Management is being performed by using evidential guides. The contents of program are created by considering the needs of patients and physicians and the services are rendered by Medical Center’s experienced healthcare professionals. The patients are informed about the visit programs and active participation of the patients in the process is maintained. The objective and the manner of presentation of service are shared both with the patient and with the physician on regular basis and the suggestions, expectations and patient feedbacks are reported regularly.

The data collected at the center by means of technological infrastructure are transmitted to the related departments statistically in compliance with the patient rights and the codes of conduct. Such reports contribute to the strategy formulation and positioning studies. Disease Management supports and improves the service provision processes in order to obtain better health results. By that way, better clinical results can be obtained, healthcare expenses can be reduced and managed better.
